PCN Pharmacy Team

The PCN Pharmacy Team can support practices & patients with structured medication reviews, medication clinics and medication queries. Patients can also be referred to the Community Pharmacy Scheme (CPCS) for minor ailments.

Occupational Therapists

The PCN Occupational Therapists specialise in promoting independence and enabling people to carry out their daily tasks. We can support with memory assessments, low level mental health issues and managing long term conditions such as pain and fatigue, to name a few.

PCN Link Workers

Providing individual support for patients as well as connecting people to community groups and agencies for practical and emotional support for a range of issues including housing, benefits, money, social isolation or anything else.

Health & Wellbeing Coaches

The health and wellbeing service uses health coaching via a what matters to me approach to support patients with behaviour change and empower them to self-manage their LTC or other health condition. Help people gain and use knowledge, skills and confidence to become active participants in their care so that they reach their self-identified health & wellbeing goals.

HCA (Mental health focus)

Supporting practices to complete the physical health checks for SMI patients and coordinating this work for practices.

Linking Leeds Social Prescribing

Connecting people to community groups and agencies for practical and emotional support for a range of issues including housing, benefits, money, social isolation, or anything else. One off or up to 12 weeks support.

Sexual Health

Offers under18s drop in, STI self-testing postal kits, emergency contraception, contraception, chlamydia testing, Pregnancy testing as well as support & advice

Citywide (LCH) Dressings clinic

Offers wound care clinic appts for assessment (60 min appt), negative pressure dressings (40-60mins), compression hosiery (20 mins), leg ulcer management (1leg-40min, 2 leg-60 min) simple wounds (20 mins) & complex wounds (40 min)
