What is a Primary Care Network?

A primary care network is a group of practices who work together to focus on local patient care. They’ve been around since 2019 and 99 per cent of practices across England now work as part of one.

With increased demand and a shortage of clinical staff, practices find they can provide greater, more personalised care for patients when they can work together on certain services or issues.

Occupational Therapists Team

Occupational Therapists (OT) work with people whose health condition(s) or the effects of aging are preventing them from being able to do the things they want and need to do.

Occupational therapist’s work holistically. They will find out what someone wants and needs to be able to do and then consider if the environment (home, workplace, school, etc.) supports or hinders their ability to do those things. They can then find ways to make life easier and support them to manage daily tasks and activities.

This might include support with self-care, household chores, getting out and about, finding or staying in work or education, or taking part in social and leisure activities.

Interventions may include adapting the environment, providing assistive equipment or technology, education around self help techniques to manage their symptoms, alternative ways of doing things, carer support and education, support in accessing community resources etc.

Occupational Therapist

I qualified as an Occupational Therapist at York St John University in 1998 and have worked in the NHS since 1999. I worked as the clinical lead occupational therapist for older adults mental health services in Doncaster for 15 years.

I joined Chapeltown PCN in June 2022 where I enjoy working with a variety of patients in particular frailty, dementia and falls prevention.

Social Prescribing Linkworkers Team

Social Prescribing Linkworkers

Social Prescribing Linkworkers work with people who need additional support to manage their long-term health conditions. I connect people to community based social groups and signpost to statutory services like housing, financial and welfare advice.

I studied Psychology for Undergraduate degree. I have worked in Third Sector for last 16 years, coordinating projects with a focus on volunteering, family health & wellbeing, domestic violence, forced marriage, sexual exploitation and mental health.

I am passionate about working to help reduce health inequalities in the communities. 

Health and Wellbeing Coach Team

Our Health and Wellbeing coaches support patients to take proactive steps to develop the knowledge, skills and confidence to become active participants in their own care enabling them to reach their health and wellbeing goals and improve their physical and mental health.

Health and Wellbeing Coach

I am a Registered Associate Nutritionist who has previously worked in Diabetes Prevention and Diabetes Support Education. Diabetes is my area of interest.

I am passionate about reducing health inequalities that exist in healthcare to help individuals lead a healthier, happier lives.
