Health & Wellbeing Coach

Patient presented with multiple health conditions; chronic pain, high blood pressure, history of strokes and Type 2 diabetes. She was a frequent attender at the practice. Health and wellbeing coach supported to set goals to promote self-management of health. Patient improved HbA1C reading from 56mmol to 49mmol.

Patient Feedback:

Patient has found the Health and Wellbeing team to be incredibly helpful in managing her healthcare needs. Their fortnightly check-ins provide a space where they listen to her concerns and offer tailored advice. She said one of the most valuable aspects has been supporting to arrange medical appointments. Additionally, they sometimes attend appointments with her, advocating for her needs and ensuring she understand the information during and after the consultation.

Going forward, she hopes to access additional support for her diabetes, improved her understanding of the condition and have control over her health.

Reason for referral:
Hypertension


Long-term goal:
To become more active and make other lifestyle changes in the hopes of lowering their blood pressure



Resources given/signposting:
Blood pressure leaflet; List of physical activities in the local area; Feel Good Factor what’s on guide; Weekly activity tracker and questions.



Short-term goals achieved:
1. Attending a Zumba class regularly
2. Walking regularly
3. Spending time in the garden and engaging in projects (helping to increase movement)
4. Wearing a step counter watch (counting steps to help motivate/track)
5. Healthy eating (reduced sugar and focussed on a healthy balanced diet)



Outcomes:
1. Blood pressure reduced (to within normal range)
2. Weight loss (patient lost half a stone)
3. Patient noticed their fitness levels had improved
4. Patient felt better in themself and more positive



Patient feedback: “Coming here has helped me so much, Sarah. You have helped me so much, you introduced me to Zumba and made me think about my lifestyle. I started coming to see you with an aim to get my blood pressure down and have I achieved that? Yes, I have! So, I feel like this service that the NHS are providing is brilliant! Sometimes people come here, and they can have a chat and talk about this that and the other, it is nice – just to come and chat, it helps with anxiety. When I come to see you, I’m not thinking about that, I feel much more positive. I’m going to do this because it is helping me. All you can do is to keep looking after yourself and do the best you can”.
